Commingling Authorization

I authorize Train & Tread Dog Co. to allow my dog to interact with dogs from other households during supervised training or exercise sessions when appropriate.

I understand that dogs over 18 months of age must be sterilized before commingling with dogs from other households.

Liability Waiver

I understand that participation in dog training and fitness activities involves inherent risks. I certify that my dog is healthy enough to participate and that I am responsible for providing accurate information regarding my dog's health and behavior. I voluntarily assume all risks associated with participation and agree to release and hold harmless Train & Tread Dog Co., its owners, employees, and representatives from any claims, injuries, damages, or losses arising from participation in services.

Veterinary Care Authorization

If my dog becomes ill or injured while in the care of Train & Tread Dog Co., I authorize Train & Tread Dog Co. to obtain veterinary care when reasonably necessary, including emergency treatment if I cannot be reached promptly. I understand that I am responsible for all veterinary expenses incurred on my dog's behalf.

Train & Tread Dog Co. will make reasonable efforts to contact me as soon as possible regarding any illness, injury, or emergency.

In the event of my dog's death while in the care of Train & Tread Dog Co., I authorize Train & Tread Dog Co. to transport my dog to my veterinarian or, if unavailable, to an appropriate veterinary facility. Train & Tread Dog Co. will make reasonable efforts to contact me promptly and will not authorize cremation or other final disposition without my consent, unless otherwise required by law.

Cancellation & Rescheduling Policy

We understand that schedules change! If you need to cancel or reschedule your appointment, please provide at least 24 hours' notice.

Appointments canceled or rescheduled with less than 24 hours' notice, as well as missed appointments, may result in the forfeiture of that session or a charge equal to the session price.
